Fantastic Jess! Just like in the greater cult, there are layers upon layers of people who protect those at the top(s) of the pyramid - through their complicity byway of different degrees of being collateralized and committed. Once all in it is difficult to get out. One image that comes to mind is how the penismastermind would sit in his chair in front of his hundreds of minions and eat one, single strawberry over the course of an hour, with each nibble eliciting gasps from some of the 'given & taken' in attendance.
